It kind of makes me wonder why he doesn't just take Shra and run, leaving Azhure behind. It's harsh, yes, but he has no reason to think the villagers would hurt Azhure, whereas if they caught him and Shra, there's no telling what they would do this time. It's true that she helped you escape, but this is your life we're talking about, and Shra's. If he's so much faster than Azhure, then he should go.
Or, alternatively, Azhure should see that she's slowing him down and insist that he leave her behind so he and Shra don't get caught again. That would be a truly selfless move on her part.

I guess that she expected the wound not to pain her at all once it subsided at first... but she should be quite familiar with this by now. So Douglass probably put it in to give Azhure some angst, not thinking about how very weird it sounds.
...did we have a timeskip here, or is Douglass so bad at showing the passing of time that she makes it sound like you could throw a rock from the village and hit the Avarinheim? They were traveling for like two minutes!
It might be my phrasing, as the book uses "After what seemed like an eternity of treading carefully". ...Not that that makes it sound especially long, either. There is no mention of the sky lightening further, for example, even though that was important to them.

Yes, I would expect this "last patch" to be relatively far away from the others and thus easy to miss. He would also be standing on dry ground with one foot and he is not going very fast. It would be possible, but it is quite contrived and not at all necessary.
Said nepotism came back to bite him in the ass very quickly, given what a bad leader Axis is. It would be hilarious if it weren't so frustrating and rage-inducing.
And, eventually, Axis will even throw him in prison! (If I remember that right, at least.) Not putting such a bastard as Axis might just have served him better.
